Who do I contact to see if a provider or medical facility is in the OCHN Network?

You can search the OCHN updated directory online, or you may call our office at 270-691-8020. If the provider is not listed in the OCHN network you may complete a provider nomination form and OCHN will contact this provider. Please note that if the physician agrees to join the OCHN network, credentialing may take up to 90 days.

Who do I contact for benefit and deductible information?
